To exchange Rs 2,000 notes at a bank, you will need to show your original ID. This ID can be any one of Aadhaar Card, Driving License, Voter ID Card, Passport, NREGA Card and Population Register.

At the top of this form, you have to write the name of the bank branch where the notes are being exchanged. Then enter the bank account number if you have it. After this you have to write your name. A photocopy of any one ID card has to be attached. ID card number has to be written. After this, the number and value of 2000 note will have to be entered in the form. You must sign the form. After this the date and place have to be written.
